「ダヴィンチ」使用手術で患者死亡 執刀医がミス 大阪・吹田市民病院
大阪府吹田市の市立吹田市民病院は19日、2020年10月に実施した手術支援ロボット「ダヴィンチ」を使った肺がん患者（60代男性）への手術で医療事故が発生し、患者が死亡していたことを明らかにした。
執刀医が鉗子（かんし）を動かした際に大動脈を損傷し大量出血したことが原因。
ロボットの不具合ではないという。遺族との和解が成立している。鉗子は部位を挟むのに使う金属製の医療器具。同病院によると、患者は20年9月、肺がんと診断された。同年10月27日にロボットを使って肺の一部を切除する内視鏡手術を受けた際、鉗子で大動脈が損傷して大量出血し、17日後に低酸素脳症で死亡。執刀した外科医が、鉗子をカメラで映る範囲外に無意識に動かした時、大動脈に接触させてしまったという。
「ダヴィンチ」は人の手より繊細な動きができるとして注目されている手術支援ロボット。同病院では18年に導入し、22年4月末までに138件の手術で使用した。事故が起きた手術は、この外科医にとって25件目だった。
同病院は事故調査委員会を設置し、21年6月に報告書をまとめた。22年1月に遺族との間で示談が成立し、その後、公表の同意も得られたという。
同病院は「患者さまとご遺族へ深くおわび申し上げます。病院全体で医療安全の確保に努めたい」などとコメントした。【三角真理】

Intuitive Surgical (NSDQ:ISRG) said this week it inked a $42.5 million agreement to settle class action lawsuits against it.
The cases, which date back to 2013, involved shareholders who purchased stock between February 6, 2012 and July 18, 2013. Shareholders claimed that the company and a select number of its officers violated federal securities laws by “making false and misleading statements and omitting certain material facts in certain public statements and in the company’s filings with the SEC,” according to Intuitive’s 10Q filing.
The plaintiffs sought class certification in September 2015, which was granted in December 2016. Intuitive Surgical sought to dismiss the complaint last year, but was denied. A trial in the case was originally scheduled to begin this October, according to the 10Q filing.
The Sunnyvale, Calif.-based company said that the settlement agreement is subject to approval by the US District Court for the Northern District of California, according to an SEC filing.
In February, Intuitive Surgical revealed a nearly $12 million legal settlement covering a slew of cases brought by investors alleging that the robot-assisted surgery pioneer misled them.

Criticism
While the use of robotic surgery has become an item in the advertisement of medical services, there is a lack of studies that indicated long-term results are superior to results following laparoscopic surgery.[7] Critics of robotic surgery assert that it is difficult for users to learn.[3] The da Vinci system uses proprietary software, which cannot be modified by physicians, thereby limiting the freedom to modify the operating system.[4] Furthermore, its $2 million cost places it beyond the reach of many institutions.[8]
The manufacturer of the system, Intuitive Surgical, has been criticized for short-cutting FDA approval by a process known as "premarket notification," and instead of entering the market through the 510(k) process. The company has also been accused of providing inadequate training and encouraging health care providers to reduce the number of supervised procedures required before a doctor is allowed to use the system without supervision.[9]
There have also been claims of patient injuries caused by stray electrical currents released from inappropriate parts of the surgical tips used by the system. Intuitive counter this argument by saying the same type of stray currents can occur in non-robotic laparoscopic procedures.[10] A study published in the Journal of the American Medical Association found that side effects and blood loss in robotically-performed hysterectomies are no better than those performed by traditional surgery, despite the significantly greater cost of the system.[11][12] As of 2013, the FDA was investigating problems with the da Vinci robot, including deaths during surgeries that used the device; a number of related lawsuits were also underway.[13]
